About Ohio Care Check
Ohio Care Check is an independent nursing-home data and consumer-information publication produced by AvW Consulting sro.
Our purpose
We organize public nursing-home records so Ohio residents, families and advocates can compare facilities and ask better questions. The directory combines ratings, staffing, ownership, inspection and enforcement fields from supplied Centers for Medicare & Medicaid Services datasets. We explain what the fields mean and retain source and processing dates where available.
Ohio Care Check is not affiliated with CMS, the State of Ohio, a nursing home, an insurer or a law firm. We do not treat a CMS rating as an endorsement or guarantee. We do not describe an allegation as proven, and we distinguish an official inspection finding, administrative penalty, settlement and court judgment.
Publisher and editorial responsibility
AvW Consulting sro is the publisher and is responsible for the site’s data presentation, original guides, corrections process and advertising separation. Guides are credited to the AvW Consulting sro Editorial Team. At this stage no individual medical or legal reviewer is represented; the site will not invent credentials. Material requiring licensed Ohio legal review must be approved before publication or activation.
How we build facility pages
The August 2026 provider file supplies the facility identity and current summary measures. Related ownership, penalty, health-deficiency and fire-safety rows are joined by CMS Certification Number. County and facility comparisons are calculated from the same Ohio snapshot. The methodology page explains dates, labels and limitations.
Editorial standards
- Identify source organizations, datasets, retrieval dates and effective or processing dates where supplied.
- Use neutral language for facilities and clearly label official findings.
- Explain missing values rather than converting them to zero or a negative conclusion.
- Keep advertisements separate from facility ordering and editorial analysis.
- Link to official Ohio and federal resources for current rules and complaint routes.
Corrections policy
If the site transcribes or displays source data incorrectly, send the page URL, field and supporting source through the contact form. The publisher will compare the report with the source, correct verified display errors and update affected generated pages. Requests to change an official CMS record should also be directed to the source agency. Do not submit private resident information.
Funding and advertising
The site may be supported by clearly labeled advertising or sponsorship. Payment cannot alter CMS data, ratings, facility order or editorial conclusions. No law firm is currently enabled in the public configuration. Read the advertising and sponsorship standards for the activation safeguards.
